Dunno about the /var/lock/qmail-smtpd dir; I don't have one of those.
With qmail successfully running, you should have four qmail-related
processes up:

qmail-send
qmail-lspawn
qmail-rspawn
qmail-clean

These you start with /var/qmail/rc.

As for the "can't read todo" error, make sure the /var/qmail/todo dir
exists and is readable by qmailq/qmail.

Are you able to inject a message into the queue using qmail-inject?
