There are step-by-step instructions at http://www.lifewithqmail.org/.  LWQ 
is available in a variety of formats.  It describes in detail installing 
daemontools with qmail and getting rid of inetd for qmail's use.  From 
there, you can figure out how to use daemontools to completely elimiate 
inetd.  (Note that apache under high loads shouldn't be run under inetd 
anyway, so daemontools wouldn't be a good option for apache either in the 
setup you've described.  Use apache in daemon mode instead.)

Note that you should have the qmail source code around.  A number of 
problems can be caused by invalid permissions on files, especially if 
you've moved the tree around some.  "make setup check" in the qmail source 
directory helps this some.
