It depends on the card... I know there are some cards that have two analog ports.  Another option is two video cards.  On this system I have a couple of older video cards, each running to a 17 in monitor.  After turning on the Xinerama extensions in X, I now have one 2048x768 desktop.  On one display I have this mutt session and XChat... on the other I have XMMS, GAIM and Konqueror going.  I have another machine for gaming, so I've not messed with any fancy 3D stuff.
