I am using ISPWest ($13.95/month if you go month to month).  They have a free 15-day trial, if this is a one shot.  No special software needed.  Standard DHCP clients, and some kind of PPP dialer for 
your modem (I have been using wvdial for the last 6 months or so).  Nationwide, don't know what cities they don't have access in, but this is info on their websited.
