Updated status:

It turns out to have been the SCSI controller.  The controller module gets 
loaded during the install and while running the rescue option from the 
install CD so the SCSI drive is accessible.  However, from a standard boot 
it locks up when mounting the file systems for R/W.  I think, but I'm not 
100% sure, that the aic7xxx modules are not getting installed.  I found 
that /etc/module* files did not have any references to the SCSI host 
adapter.  I added the references manually, but that didn't help.  I forgot 
to check if the modules were actually installed before I rebuilt the 
system w/o the SCSI card installed (oops).  After rebuilding the system 
w/o the SCSI controller everything came up as it should.

BTW, this is an Adaptec 29160N controller which the MDK hardware database 
says is supported.  I know it's supported, you just can't install it ;)  I 
have to defer solving this to a later date.  If anyone else finds a 
solution to this please post it, MDK has not gotten me an answer yet.
