I should have mentioned that.

I booted the first CentOS-4.0 CD and entered 'linux askmethod' at the
prompt (then selected NFS).

Actually, the bios on that machine is too old to boot from CD, so I 
had one more step.  I booted with a Smart Boot Manager diskette
<http://btmgr.webframe.org/>, then told it to boot from CD.
