[NTLUG:Discuss] survey using PDF with fields

Hello all,

I'm helping out my local neighborhood association put out a  
questionnaire.   My idea would be to send out a PDF file with fields  
that people could fill in, save, and then e-mail back.  Anyone know  
if there is any Open Source software that would allow me to create  
this?  Can it be done at all?

Our neighborhood group has a sizable number of households (~3,000) to  
whom this would be sent.  So this would be a high-visibility item for  
Open Source.
