hdparm didn't have any problems.   Did you check these?

1. smartctl (from smartmontools) and see if the drive is detecting internal
errors.
2. disable autofs (and remove the module)
3. try it in single-user mode
4. enable full debugging in syslog, and see if any errors were going
unreported
5. boot with "pci=noacpi noapic"?
