Hi Cody.

What you are looking for is called Identity Management & Provisioning.

If you are looking for a large scale solution, I am honor bound to 
suggest HP's Select Identity.  Does what you want, as well creating and 
provisioning the user account(s) automatically on the appropriate 
systems (such as your email system).  Can also be used to modify a user, 
allow a user to change all of their passwords in one go, and 
disable/terminate all user accounts at once as well.  Currently supports 
DB2 & Oracle for the backend databases, and as it is Java Based, 
supports Linux & MS for the OS.  Other combinations are available on 
request.

Version 4.0 is being released today.

The competing projects to this, while they in general tend to be easier 
to set up, also tend to be much more limited in how flexible and 
expandable they are.

Disclosure Statement:  I have been kept happily overemployed for the 
last 18 months putting in new features (as well as some cleanup of old 
features) working as one of the Developers on SI.

If you want to "Roll your own" solution, you might want to have a look 
at the Keel MetaFramework (http://keelframework.org).  Then, you would 
only have to create some UI pages (Keel supports several, including JSP, 
Tiles, and Velocity, among others), as well as some Services, and you 
would be on your way.
