Yes, this is a brand new, never before formatted by me, disk until the Linux
format.

That sure screams of a file system corruption....but on a freshly
formated partition??
I'd run a destructive disk test...and maybe change cables and/or
controller or port.
Is this a brand NEW disk -- as in never before used??
