I was very frustrated when I had similar symptoms.  Your process is very good.

I found two issues:
1. Scratched or dirty media.  If it was scratched, I shredded it.  If it was dirty, I used an 
eyeglass cleaning kit on it.
2. Bad memory.  This will cause a lot of different, and strange issues.  Most distro media will 
contain an option to test memory, (in addition to installing the distro).  Start the memory 
test when the system is at room temperature, leave it off over night.  If it passes initially, 
just let it continue the test as it heats up.
