The answer is simple, which seems to make it difficult to comprehend?
No modem ... FiOS ended in my house as an Rj45 jack installed in the 
wall.  It is in fact and Ethernet connection and so, again, NO MODEM is 
required.  Run an ordinary Rj45 terminated Ethernet cable from wall jack 
to the input port on your switch/router (WRT54G, is the example given, 
but others will work as well).

What happens?  Follow the set up instruction on your device, but 
generally ...
     Verizon gives you one dynamic IP ... you can waste it on one 
computer, or you can give it to your switch/router
     The switch/router gives you a private address range ... typically 
192.168.1.x ... set those to be dynamic as well
     Printers ... I have had the best luck setting those as static IPs 
within the private address range
