That card has used DEC Tulip chips in the past - but they've also used others
too.  Pretty funny that they wouldn't change the documentation.

I'm not at the NIC now to look at the chip, but the weird thing is that
the documentation says to use the tulip module yet `insmod tulip` gives
me "Device or resource busy." (worthless error msg)

I'm going to try the append="ether=0,0,eth1" and see what that does.
