This is what I use to dial in using wvdial:

[Dialer Defaults]
Username = indraneel
Init1 = ATZ
Init2 = ATQ0 V1 E1 S0=0 &C1 &D2 +FCLASS=0
modem = /dev/modem
Password = xxxxxxxx
Modem Type = Analog Modem
Area Code = 214
Dial Command = ATDT
Tonline = 0
Phone = 4463481
Baud = 115200
Auto DNS = 1
Stupid Mode = 0

The above works perfectly everytime. I should add that setting up the
account on Windows took nearly half a day, and wvdial configured
everything in about 10 seconds (I had to supply the phone number, username
and password). And the authentication is not PAP/CHAP.
