"a vertical rectangle about 1/2 inch by 1 inch attached to the mouse"? 
 Are your talking about some sort of distortion on the screen next to 
and/or following the mouse cursor?
If so, sounds like either display settings in XF86Config not exactly 
right, or you could have a hardware issue.
Have you tried a RedHat install?
You might show a portion of XF86Config file here and let some of us look 
at it.
