Have you looked on www.freshmeat.net for 'ticket' projects? 

I set up and demo'd PHPTICKET last year ( the client didn't
want to go with a ticket system so I didn't persue it any
farther than the demo ) but from what I recall, it did 
everything that you listed below. 

There are several 'ticket' programs on freshmeat.net, you 
might want to take a look at them. 

I could probably set up a few of them on my test system
if you wanted to give them a spin.
