We are a small ISP.  We enforce our AUP very strongly
and do not have a spam problem.

I have not encountered an ISP outside the US that takes
spam as a problem.

Some ISPs in the US are very good.  This is mostly the smaller
ones.  I have worked with several of them that changed their 
reverse DNS schemes to better accommodate our filtering.

Most ISPs in the US (especially the larger ones) try to make an 
effort but are ineffective because they cannot police their 
network with the manpower allocated to the task.  This is an 
allocation problem. If they want to offer email services, they 
need to make sure to allocate proper resources to policing it.
