I had a Toshiba laptop and it required having the mixer set.  There is a 
mixer program "mixer", command line only that I installed and used it to 
set the volume to a usable level under /etc/rc.d/rc.local (I was running 
Slackware on it).

Toshiba's default is for the mixer to be set to zero and it isn't going 
to let you do anything different as a default. Hence, "mixer".
