On my Ubuntu/macBook the unity xrandr interface showed the outputs but I
could not use them until I ran the Nvidia control app.  It had a check box
to enable/disable the output(s).

After toggling the HDMI to "on" inside the NVidia app the HDMI out was
controllable from within the Ubuntu xrandr.  Maybe that will get you there?
