Given the politics surrounding the "blacklists," and the futility of trying to keep
out the undesirables, I've recently turned to TMDA
(http://software.libertine.org/tmda) in an attempt to have more control over who
*I* want to have access to my mailbox.  It's approach is a "whitelist" approach:
Ban (or force confirmation) on everyone but those on your whitelist.  Give
non-spammers who desire to send you e-mail the one-time option of confirming
themselves as human (as opposed to an auto-replyer). 

It's not without its glitches, but after trying the blacklist approach with little
success, I decided it was time I made the decisions, rather than someone else.
